Prof. Dr. Hasan Kocaeli Uludag University, School of Medicine, Neurosurgery Department, Bursa TURKEY A case of the piece of wood due to transorbital penetrating into the temporal lobe of the brain is demonstrated. 24-year-old man, during working, a wooden stake hit his face and stabbed his right side of the nose directly. On admission, the wooden piece could not been detected in appearance. CT scan showed traumatic subarachnoid hemorrhage on the right temporal lobe. The patient was treated for traumatic head injury at first. Later, it proved by MR images that he was stabbed with a wooden piece penetrating into the right temporal lobe through the medial side of the right globe, crossing the orbital part of the optic nerve and superior orbital fissure. Cerebral angiography was obtained to demonstrate for the possibility of traumatic vascular injury. It was normal. Transfacial approach was performed with local anestesia and piece of wood (10 cm) was withdrawn carefully without any neurovascular complication. It is difficult to confirm intracranial wooden piece accurately by means of only plain skull film and usual CT scans. It is necessary to utilize the MR images with axial, sagittal and coronal sections. |
