ON EXAMINATION SHE WAS CONSCIOUS GCS 15/15 & HAD NO NEUROLOGICAL DEFICITS HAEMTOLOGICAL WORK UP SHOWED NORMAL FINDINGS Investigated With MRI Scan Which Showed An Isodense Lesion In The Left Cp Angle Region With Clear Demarcation & Perilesional Edema . Patient Was Taken Up For Surgery After Counselling Regarding The Size And Implications Of Surgery Vs Non-surgical Tretament. Operated With Park Bench Postion Left Sub-occipital Retromastiod Craniectomy & Total Exicision Of Tumour. SURGICAL PEARLS POSITION-Use Park-bench Position With Head Above Heart Level BASIC REQUIREMENTS- Mayfields Head Clamp, Microscope , Leyla Retractors,micro Instruments And CUSA USE TOTAL INTRAVENOUS ANAESTHESIA WITH GOOD ANESTHESIC TEAM,GOOD ICU BACKUP TEAM Add CVP And IABP For Monitoring Through out The Surgery POST-OP --Patient Was Neurologically Normal And Improved. POST-OP --CT SCAN Showed Good Clearance At The Tumour Site And Resloution Of Edema. POST-OP --Patient Has Made Complete Improvement. |
