| Video Discription | Problem: Elderly patient with multiple comorbid conditions, including HTN, polycythemia rubra vera and myelofibrosis; normal platelet count. Colonoscopy: Insertion: Sigmoid colon narrow and tortuous; took 21 minutes to reach cecum. EMR: After submucosal injection of Indigo Carmine (without epinephrine), piecemeal EMR was done with a 10 mm snare. Minor bleeding from the base was controlled with APC and clips. Followup: Uneventful. Pathology: Tubulovillous adenoma with multifocal high grade dysplasia.No cancer. Plan: Repeat colonoscopy in 4-6 months. |
