| Video Discription | This channel is dedicated for public education about colon cancer prevention. This case demonstrates some of the difficulties associated with resection of a low rectal lesion. Use a cap and keep it a few mm longer to allow better access to the lesion. There was quite a bit of rectal contractions that required patient position changes, use of glucagon, and pure patience to do the resection. It also shows that multiple injections may be required to get a good lift of the lesion because prior resection attempts resulted in scarring of the lesion. In addition, picking up the correct snare is critical: First selected 10 mm snare; then changed it to 20 mm for the first cut; marginal cut required going back to 10 mm snare. After complete resection, the edges were cauterized to prevent delayed recurrence and the blood vessels in the base were cauterized to prevent delayed bleeding. Clip closure was deferred as the lesion was close to the anal canal. |
