| Video Discription | This is a robotic portal VATS (video assisted thoracoscopic / thorascopic / transthoracic) diaphragm plication utilizing a the davinci XI for symptomatic chronic right phrenic nerve injury from trauma. 3x 8mm Robotic Ports, 1x 12mm Airseal Port. 1) Start with a 18" pledgetted #1 V-Loc suture to take the most tension off of the plication and facilitate subsequent sutures. (Bring diaphragm together perpendicular to fibers.) 2) Pledgetted 2-0 Ethibond Horizontal Mattress Sutures. Use many. 3) Running #1 V-Loc suture to reinforce plication. Postoperative CXR showed nearly doubling in size of right thoracic cavity. Marked improvement in subjective breathing. Blake drain removed next day and discharged home. Plan for PFT's in 6 months for new baseline. |
