Frozen abdomen making transabdominal approach impossible. Notes: Right sided approach, 3x 8mm ports with 12mm AirSeal Assist (8mmHg). Multiple divisions of vagal trunks, all divided. Clips lefts as markers on CXR. Left pleural space entered and widened to minimize tension pneumothorax. Blake drain crossing into left pleural space, chest tube to apex. Has not had upper GI bleed since recovering from procedure. Pathology confirmed nerve tissue. No chyle leak. |
