ACom aneurysms are the most common brain aneurysms, accounting for 30-35% of aneurysmal subarachnoid hemorrhage. Small aneurysms in patients without risk factors (high blood pressure, smoking, worsening headaches...) can be followed. Larger and irregularly shaped aneurysms, especially in patients with risk factors, need to be treated. Options include endovascular coiling with or without stenting and craniotomy for microsurgical clipping. This video illustrates surgical clipping of an unruptured 1.5 cm aneurysm via standard pterional craniotomy using sequential remodeling of an aneurysm sac with clips placed in a "picket fence" fashion. |
