| Video Discription | Hepatic TACE is the treatment of choice in patients with intermediate stage HCC, according to the Barcellona Clinic Liver Cancer (BCLC) staging system. TACE procedure consists in the placement of a micro-catheter in the arteries feeding the tumor for direct locoregional infusion of chemotherapic drugs into the tumor, reducing systemic toxicity. In DEB-TACE the chemotherapic drug is delivered to the tumor into microspheres that increase the effect of drug both inducing tumor ischemia (vessel occlusion) and prolonged slow delivery. PROCEDURAL STEPS AND TECHNIQUE After local anesthesia administration, a percutaneous femoral artery access is obtained via Seldinger's technique. A vascular sheath introducer (usually 5 French sized) is inserted into the femoral artery and a diagnostic catheter is advanced on a guidewire across the aorta and placed into the common hepatic artery (according to anatomy). A diagnostic angiography is obtained to quantify tumor burden and plan the treatment strategy. A micro-catheter is coaxially inserted into the diagnostic catheter and advanced to the target vessel. Drug infusion is finally performed with multiple possible techniques. 1. Conventional TACE: in cTACE a mixture of Lipiodol, a carrier molecule which brings the drug into cancer cells, and an adequate amount of drug is composed before the procedure. To follow, particles are injected to induce cancer ischemia. 2. DEB-TACE: in DEB-TACE Lipiodol is replaced by tailored particles which have the double capacity of bring the drug into the tumor and induce ischemia. 3. DSM-TACE: DSM-TACE is similar to DEB-TACE but particles are adsorbable. 4. bTACE: recently introduced for the treatment of large nodules, bTACE consists in performing TACE with a microcatheter which has a small inflatable balloon on its tip. Performing TACE with this device increases the amount of particle and drug which reach the tumor inducing flow redistribution from normal liver parenchyma to the tumor itself. Side effects Patients undergoing TACE can experience side effects. A small amount of symptoms such as astenia, feeling or being sick, abdominal pain and increased body temperature are included in that which is called post-embolization syndrome and usually resolve in few days. Other possible side effects include: - Infection/abscess in the treated area due to the induced ischemia (treatment should always be as selective as possible!) - Allergic reaction to the contrast medium - Non-target embolization (drug and particles reach district other than the liver and the tumor) - Vessel catheter-induced injury (dissection or rupture) - pain in the groin region La TACE (Chemioembolizzazione trans-arteriosa) è il trattamento di scelta nei pazienti con HCC in stadio intermedio, secondo il sistema di stadiazione Barcellona Clinic Liver Cancer (BCLC). La procedura consiste nel posizionamento di un piccolo catetere (microcatetere) nelle arterie che alimentano il tumore con successiva infusione locoregionale di farmaci chemioterapici direttamente all'interno del tumore stesso, riducendo significativamente la tossicità sistemica epatica e sistemica.
