| Video Discription | Colloid cysts are usually a well -defined round or oval non-enhancing lesion. MRI signal intensity is variable, with any combination of T1 and T2 signal intensities, the most common appearance is hyper-intensity in T1 weighted sequences and iso to hypo-intensity in T2 sequences. Low-signal cyst on T2- weighted sequences are difficult to aspirate (high viscosity) whereas majority of high-signal lesions are easy to aspirate. Only clinical follow up is needed when risk score is 2 or less. Clinical and radiological follow up is needed when risk score is 3. Surgery is indicated if risk score is 4 or more, or in presence of hydrocephalus. Channeled Endoscopic technique is an effective alternative to microsurgical technique. There are difficulties in bimanual dissection of the cyst wall, and control of hemorrhage especially when lesion is T2 hypointense or CT hyper dense and those placed posteriorly in third ventricle in channeled endoscopy. Port or tubular retractor is especially useful in this group. Total tumor excision and control of bleeding can be better achieved by endoscopic tubular than by channel endoscopy. Although total excision should be aimed, residual cyst wall should be coagulated in subtotal excision cases. It usually remains asymptomatic. There is steep learning curve in endoscopic surgery. |
