| Video Discription | Etude 28. Temporobasal trigonal tumor 60 y,m, with two-year history of lung cancer. Metastasis in the region of right trigonum with mediobasal growth. Patient had normal vision preoperatively. I had two options regarding approach: trancortical via lower temporal sulcus and subtemporal. I prefered focused transsulcal approach for better visualization of cranial part of the tumor. After debulking of the mass I encountered lat. posterior choroidal artery which crossed the tumor. After dissecting this vessel more radical removal of the tumor could be achieved. Postoperatively, patient had small visual field deficit and some visual halucinations (due to corticoids, I guess). Both symptoms resolved completely within one week after surgery. Could walk without aid. The first postop MRI showed total removal of contrasted tumor and no complications. Unfortunately, only after 6 months patient developed multiple metastasis despite of X-ray/chemotherapy. This indicated switch to palliative treatment. |
