| Video Discription | Right side, midde fossa (subtemporal) approach was used to enter the middle cranial fossa and identify the defect of the tegmen tympani form above. This patient had recurrent attacks of meningitis. Coronal HRCT scan revealed a bony defect at the level of the posterior tegmen. Since there was good hearing in that ear, transmastoid approach for the reconstruction ( removal of the ossicles and repairing the defect from below, with a blind sac closure of the external ear canal after mastoid cavity obliteration) was not preferred. In contrast, a subtemporal cranitomy was performed (middle fossa approach) and the defect was identified, as well as the geniculate ganglion, head of the malleus, tensor tympani tendon and tensor fold from above. A three layer closure composed of fascia, tabular bone graft and muscle layers was used to reconstruct the defect and tissue glue was used to help the layers held in position. Note that, endoscopic transillumination was used to identify the defect region; after removing the abnormal bony fragments around the defect, endoscopy was also used to explore the epitympanum from above to search for any diseased neural tissues (meningocele, encephalocole, etc). Postoperative coronal HRCT one day after the surgery revealed the complete closure of the defect. |
