| Video Discription | In this video, I review the surgical management of a pituitary adenoma involving the infundibulum (pituitary stalk), approached via an endoscopic endonasal route. Preservation of the pituitary stalk is a key objective to maintain hypothalamic-pituitary axis function and reduce the risk of postoperative diabetes insipidus or hypopituitarism. Lesions involving the stalk present a unique challenge, as distinguishing tumor from normal stalk tissue intraoperatively is often difficult. Recent literature suggests that subtotal resection with stalk preservation can still yield favorable hormonal control in functioning adenomas, while minimizing long-term endocrine morbidity.
