| Video Discription | This video demonstrates the repair of a type 3 paraesophageal hernia repair with mesh done robotically (da Vinci Xi). First the herniated stomach was reduced back into the abdominal cavity . Small gastric vessels were taken down along the greater curvature. The lesser omentum was opened, giving a clear view of the caudate lobe of the liver and inferior vena cava. Next, the hernia sac was dissected off the right and left crura. A penrose drained was encircled on the esophagus to provide gentle traction during mediastinal dissection. Sufficient mediastinal dissection was performed for there to be 2-3cm of intraabdominal esophagus. The hiatus was repair with 2-0 Prolene barbed sutures. The repair was reinforced with absorbable mesh. A partial/ toupet fundoplication was perfomed. Completion endoscopy confirmed integrity of the operation. |
