| Video Discription | There are multiple scalp incisions available for access to the retromastoid area. Here we show exoscopic 4k video of the scalp and muscle dissection we prefer for retrosigmoid approaches to the CP angle. In this approach(which I learned from Laligam Sekhar MD at UW), the scalp is elevated off the suboccipital musculature. The SCM is elevated up with the scalp flap separately from the fascia covering the splenius muscle. The splenius and suboccipital muscles are then elevated in a subperiosteal fashion and reflected inferiorly. Minimal muscle cutting is required. In thin patients, virtually no muscle cut is required. With bigger muscle flaps, a small incision along the posterior edge of the muscle(from the posterior portion of superior nuchal line down towards foramen magnum) is sufficient to provide the inferior access required. In my experience, this is superior to alternative muscle cutting approaches in that the postoperative neck pain is minimal in comparison. In addition there are no scalp retractors to contend with during the operation thereby improving operative ergonomics. |
