| Video Discription | Sphenopetroclival meningiomas are a specific type of meningiomas that can invade the parasellar region of the middle fossa via the trigeminal porus. We present the surgical approach in such a case. The patient was fully neuromonitored during the operation.The suboccipital retrosigmoid approach made it possible to reach the posterior petrous, clival and subtentorial regions while preserving the cranial nerves. Tumor mass and dural attachments can be devascularized under direct view of the petrotentorial surface. With this approach, the risk of injury to the 7th and 8th cranial nerves and the trigeminal nerve is kept to a minimum. Tumor resection was achieved in this patient without additional neurological deficit. The middle fossa extension of the tumor was left as a residue for stereotactic radiosurgery. Sfenopetroklival menenjiyomlar, trigeminal porus yolu ile orta fossada parasellar bölgeye invaze olabilen spesifik bir menenjiyom türüdür. Böyle bir olguda cerrahi yaklaşımı takdim ediyoruz. Hasta operasyon süresince nöromoniterize edildi. Suboksipital retrosigmoid yaklaşım, kraniyal sinirleri koruyarak posterior petröz, klival ve subtentoryal bölgeye ulaşmayı mümkün kılmıştır. Tümör kitlesi ve dura ataşmanları, petrotentorial yüzeyin doğrudan görünümü altında devaskülarize edilebilir. Bu yaklaşımla 7. ve 8. kranial sinirler ile trigeminal sinirin yaralanma riski minimumda tutulmuştur. Bu hastada ek nörolojik defisit olmaksızın tümör rezeksiyonu sağlandı.
