| Video Discription | To get in touch drop a mail to - drpavaman@gmail.com Ventral herniation of the thoracic spinal cord is a very rare condition. A total of 180 cases worldwide are reported. We present one such case. The most common differential diagnoses are arachnoid cysts and arachnoid webs. The differentiating feature is the CSF flow study across the suspected area. CSF flow is unhindered in cord herniation and turbulent or non-existent across the arachnoid webs. Repair of defects can be done with artificial dural substitutes. We used a muscle/fat patch and stitched it across the defect. Even with neuromonitoring post-operative chance of paraparesis/plegia is high as 50%. Our patient was discharged uneventfully on POD 10. The wound was healthy and preoperative paraparesis was improving with the patient ambulating with minimal/occasional support of one person. |
