The dissection begins on the dorsomedial side of the thigh. The posterior part of the iliotibialis lateralis muscle, which covers the entire dorsum of the thigh is dissected as well as the femorotibialis internus muscle. Dissection is carried out in the fasciae between the muscles, with careful separation and care given not to cut any vessel. If it is compulsory, the vessels should be ligated before cut, to simulate live animal experience. The neurovascular bundle is discovered in a layer of fatty tissue close to the femur, lying on the two pubo-ischio-femorales muscles (lateralis and medialis) and the flexor cruris lateralis and medialis muscles. The magnification is then set to 20x and the nerve minutely dissected from all its coverings, all the way to the epineurium with extra care dedicated to not damage any of the delicate tissue. The nerve is then cut in one sharp move with the microscissors, and the sensory (distal, smaller) and motor (proximal, larger) bundles are identified. At this point we move to the second part of lesson 4, the vascular anastomoses and to lesson 5, the neurorrhaphy. More videos to come! Song: Bach Goldberg Variations.
