| Video Discription | Minimally invasive transcranial surgery techniques have provided alternative operative corridors for exposure of complex skull base lesions including aneurysms. Although these minimally disruptive approaches have gained popularity for management of tumors, their use for clip ligation of complex aneurysms is debated. The potential advantages of supraciliary incision and supraorbital keyhole craniotomy include minimal exposure of the normal brain tissue, decreased frontal lobe retraction, and improved cosmetic result. In the following video, the author describes the use of the above keyhole approach for clip ligation of a 10mm broad-based unruptured anterior communicating artery aneurysm in a 62-year-old patient. Preoperative CT angiography demonstrated a dominant left A1 branch and an anterior communicating complex that had rotated toward the right side (away from the operative view). Through a supraciliary incision, a left-sided supraorbital craniotomy was conducted and the frontal sinus was avoided. Gentle elevation of the frontal lobe and dissection of the anterior and medial aspects of the Sylvian fissure allowed an excellent exposure of the aneurysm and proximal vascular control without the use of fixed retractors. Under temporary regional circulatory arrest using ipsilateral A1 temporary clip ligation, two permanent clips were placed across the neck of the aneurysm. Intraoperative fluorescence imaging confirmed exclusion of the aneurysm without any compromise of parent or branching arteries. The patient recovered from surgery without any complication. |
