| Video Discription | Lazy S incision and paramedian approach have advantage over Midline approach linear incision in preventing csf leak and easier exposure and closure in posterior fossa. patient is old lady and midline approach would have required extreme neck flexion which is not possible in old age due to cervical spondylosis degenerative changes. 68/F, previously k/c/o Uterine carcinoma, now admitted for loss of consciousness, vomiting with difficulty in walking. on evaluation, however there were no recurrence of cancer at primary site but there were metastasis in brain, lungs, pelvic bones. patient's family chose for surgery for cerebellar metastasis. if tumor size is larger then 2.5-3 cm then surgery is required.
