| Video Discription | • Although open posterior midline laminectomy has been successfully used in intradural extramedullary, there are risks of postoperative spinal instability and deformity in long-term follow-up due to bilateral subperiosteal muscle stripping, extensive laminectomy, and partial facetectomy which may need fusion. • Other disadvantages of open surgeries are increased pain, and neurologic deterioration, increased risk CSF leak and wound infection due to more dead space. • Minimally invasive approaches with the help of microscope or endoscope using unilateral hemilaminectomy with preservation of musculoligamentous attachments and posterior bony elements have been found to be safe and effective which reduces trauma-related instability with comparable outcome. • Endoscope has the advantages of panoramic view. • It is indicated in small tumor up-to 3.0 cm. It is used when tumor is lying posterior or posterolateral to cord, and for anterior lesion after gaining experience • It has advantages of minimally invasive nature, safe and effective with comparable operative time and less blood loss. The tumors of all locations (cervical, dorsal, lumbar etc.) can be removed. There is significantly decreased dead space, decreased rate of CSF leak, and preservation of facet joints and muscles with improved spinal stability even in cases of foraminal tumors. • There are limitations such as difficulties in tumor localization in thoracic region. Removal of a large tumor involving more than two vertebral segments is difficult. There is a steep learning curve, and meningioma with very broad dural attachment are also difficult to remove. There is difficulty in removal of tumor placed purely on anterior side and difficulty in dural suturing. This difficulty in dural suturing can be overcome by using dural substitute and fibrin glue. Retract tubular retractor to get more space for suturing. Anastoclip and Covidien Endo suture are likely to improve dural suturing in future. |
