| Video Discription | Operative Video (Sound On, play at 720p HD): Removal of a spinal cord hemangioblastoma (C4/5) in a patient with von Hippel-Lindau (VHL) disease. Neurosurgeon: James K. Liu, MD, FACS, FAANS The tumor was progressively growing and causing significant swelling (edema) of the spinal cord. A C4-6 laminectomy was performed and the tumor was completely resected. The tumor is carefully and meticulously separated from the spinal cord using sharp dissection with microscissors. Care is taken to avoid damage to the spinal cord. Neurophysiological monitoring is imperative. An instrumented fusion with lateral mass screws and rods was performed for spinal stabilization. Postoperatively, the patient was neurologically intact and discharged on postoperative day 2. Von Hippel-Lindau disease (VHL) is a hereditary condition associated with tumors arising in multiple organs. Tumors in VHL include hemangioblastomas, which are benign blood vessel (hypervascular) tumors of the brain (cerebellum), spinal cord, and eye (retina). Patients with VHL also have an increased risk of developing clear cell renal cell carcinoma (a type of kidney cancer), pheochromocytoma (a tumor of the adrenal gland), and pancreatic neuroendocrine tumor. Other features of VHL include: kidney cysts, pancreatic cysts, epididymal cystadenomas (tumors near a man’s testicles), and endolymphatic sac tumors, which are tumors in the temporal bone that may cause hearing loss. VHL disease is a genetic condition that runs in families. If you have von Hippel Lindau disease, or if you have a family history of the disease, genetic counseling is recommended.
