Previous ventral hernia surgery with or without Meshplasty also play a role in decision making. Tansversus Abdominis release is usually not required in smaller defects. Poor skin with infection or with sinus tract excludes e TEP Technique: Position of patient and port placement: The position is supine with hands on the side- to give enough space for the movement of the surgeon and the assistant. Monitor is placed at the foot end. Surgeon and camera assistant stand to the left of the patient. Initially the camera is placed in the left upper port to create a Retro-Rectus space and after exposing the pubic bones and lateral dissection, the camera is shifted to the left middle port for the cranial dissection. For suturing the defect, the camera is again placed in the left upper port. Retro-Rectus space: Retro-rectus space can be created with a balloon or with 10 degree scope as is shown in the video. The dissection proceeds caudally until the exposure of the pubic bone. The lateral limit of the dissection is just medial to the linea semilunaris. Midline: Camera is shifted to the medial port on the left side to have a cranial view. The contralateral Rectus sheath is opened with the help of hook cautery and Retro-Rectus space is created. The dissection is continued till umbilicus and the hernial sac is reduced. The defects are defined. The inguinal hernia sac also dissected and the contents reduced. Adequate space is dissected for the placement of the mesh. The posterior Rectus along with the peritoneum is closed with the help of absorbable suture. Here we used V loc (Barbed) suture. Closure of the Linea Alba: Linea alba is also closed with V Loc suture. Placement and Fixation of the Mesh: Two Prolene mesh have been placed and fixed in this case with tackers. The mesh have been adequately fixed and placed before deflating. Post-operatively, patients have minimal pain and are usually allowed liquids 6 hours after the surgery. Most of the leave the hospital after 2 days. |
