In this video, Dr. Daniel Kelly, Director of the Saint John's Brain Tumor Center & Pituitary Disorders Program demonstrates endoscopic tumor removal in a young woman with a microprolactinoma. She had been treated for several years with a dopamine agonist (cabergoline) but was having persistent side effects and hyperprolactinemia (elevated prolactin). Given the unpleasant side effects of the medication and elevated prolactin, endonasal endoscopic tumor removal was a reasonable option in her case, particularly since increased prolactin can cause infertility, sexual dysfunction, weight gain and milky discharge from the breast (galactorrhea). In this case, the tumor also had invasion into the left cavernous sinus, making removal more challenging. The surgical video demonstrates how the adenoma is removed, the pituitary gland preserved and the visual benefit of the endoscopic technique for removing tumor within the medial cavernous sinus.
