| Video Discription | Abstract: Background and Introduction: Pituitary adenoma forms about 3% of pediatric intracranial tumors. Gross total resection is curative in nonfunctioning adenomas, without additional treatment. Objective: In this video abstract, we present a case of a giant pituitary adenoma invading the cavernous sinus in the pediatric age group. Surgical Technique: An 8‑year‑old girl presented with bilateral diminution of vision, and ptosis in her left eye. Magnetic resonance imaging (MRI) brain with contrast showed a homogeneously enhanced large sellar lesion with suprasellar extension, it was invading the left cavernous sinus with a parasellar extension, and it was extending up to the third ventricle. She did not have any clinical or laboratory endocrine dysfunctions. We favored the endoscopic transsphenoidal approach because the main bulk of the tumor was mainly in the midline, to achieve adequate chiasmatic and cavernous sinus decompression, and the long axis of the tumor was at the same angle of attack. Results: The patient had an uneventful recovery with an improvement in the diminution of vision and ptosis. Conclusions: The endoscopic endonasal transsphenoidal approach for sellar and suprasellar tumors is a safe and effective technique in the pediatric age group. |
