| Video Discription | A young girl presented with one episode of seizure. She underwent MRI scan which showed left frontal T2 hyperintense, T1 hypointense, non-engancing lesion with diffusion restriction which is epidermoid tumour. Epidermoid tumours are benign slow growing tumours lying in the subarachnoid spaces and known for their irritative symptoms like seizures in this case. They are avascular, easily suckable and pearly white in color. The one characteristic that makes them deadly is that they grow within the cisterns and amidst the blood vessels. This poses risk of vascular injury especially the injuries to perforator vessels at the skull base. In this case, a subfrontal skull base approach was chosen aiming at complete tumour clearance from the parenchyma, the cribriform fossa and opticochiasmal cisterns. No corticectomy was required to approach the tumour. The key to safe skull base surgery is microscopic arachnoid dissection allowing safe brain retraction, which is shown in the video. At the end of the procedure, post op MRI shows complete tumour clearance and the cavity filled with CSF.
