They carry significant morbidity and mortality and pose a significant challenge to the patient as well as the surgeon. Such fistulas lead to nutritional imbalance, fluid loss, and difficulty managing the output with skin inflammation. Surgical correction is the only effective treatment modality. Ideally, the patient should be optimized medically and nutritionally in addition to being free of any active infection prior to surgical intervention. After extensive adhesiolysis, and resection of the involved segment of bowel, the abdominal wall needs to be reconstructed. Components separation allows mobilization of the retracted edges of the abdominal wall in order to reach the midline for a closure. The repair can be reinforced with the mesh if needed. A variety of technical modifications of the original components separation have been described. In this video, Professor Maher A.
