| Video Discription | This 68-year old woman suffered from a subarachnoid and intracerebral haemorrhage from a right-sided middlel cerebral artery (MCA) bifurcation aneurysm. She presented WFNS grade III. An emergency surgery was executed. From the base of the aneurysm the intermediate artery appeared. The aneurysm was clipped and the MCA bifurcation reconstructed. The intermediate artery was then cut and reimplanted on the anterior temporal artery (M4), as such an IC-IC bypass was performed. Postoperatively, there was no sign of ischemia and the patient recovered without neurological sequelae, much to our surprise. Surgery performed by: R. Dammers and V.
