| Video Discription | In this video, microneurosurgical resection of clinoidal meningioma is presented. ICA and optic nerve were invaded by the tumor. The patient had visual defisit preoperatively. Anterior clinoidectomy and Transsylvian approach were performed. Anterior clinoidectomy provided the early and wide exposure of ICA, and the mobilization of ICA as well. Following devascularization of the tumor, optic nerve was exposed and decompressed. Circumferential decompression of both ICA and optic nerve was achieved. Preoperative visual defisit of the patient became normal. Near-total tumor resection was performed in this patient due to the local invasion of the tumor to ICA. No radiosurgery is planned for the residual tumor. It is thought that the potential second operation in the future is more effective and logical than the any type of radiotherapy due to the local burning effect of RT to optic nerve and malign transformation. |
