| Video Discription | As more aneurysms are being treated with endovascular coiling than ever before, incidences of incomplete aneurysm occlusion and recurrence are also growing. Intraluminal thrombus formation and fibrosis can adhere the coils to the aneurysm wall, preventing their removal or risking arterial tear if attempted. In this 3-dimensional video we show the case of a 34-year-old man who presented with a subarachnoid hemorrhage from an anterior communicating artery aneurysm. His endovascular coiling at an outside hospital was complicated with perforation of the aneurysm during coil deployment and extravasation of contrast. He was transferred for surgical management. Indications for microsurgical clipping included ruptured aneurysm, coil perforation with intraprocedural rehemorrhage, incomplete coil occlusion, and young age with long life expectancy. The anterior communicating artery aneurysm was exposed through a standard right pterional craniotomy. Coils were extruded into the subarachnoid space through the aneurysm dome, but coils at the neck prevented clip application. The anterior communicating artery complex was isolated between temporary aneurysm clips. The dome was opened and coils were extracted superiorly, creating room at the neck for a closing clip. The previously coiled superior lobe was occluded with a single clip, and the previously untreated anterior lobe was occluded with a series of stacked straight clips. After the temporary clips were removed, indocyanine green dye confirmed patency of the parent arteries and occlusion of the aneurysm. A right middle cerebral artery aneurysm was clipped too. The patient recovered from his subarachnoid hemorrhage and made an excellent recovery at 4-month follow-up evaluation. |
